Somewhere along the way, the appointments, medications, equipment checks, and endless paperwork took over. MyCarePlan gives you your role back.
Why This App Exists
I spent years working as a complex care nurse — visiting families in their homes, supporting children on ventilators, families managing tracheostomies, gastrostomies, multiple medications, and a revolving door of healthcare professionals, therapists, and support workers.
I watched families who were extraordinary, devoted, and exhausted. Not because they couldn't cope — but because the system gave them no tools. Parents were tracking medications in notebooks. Keeping equipment service dates on the back of envelopes. Re-explaining their child's entire history at every single appointment because no one shared records.
I watched mothers break down in tears — not because they couldn't care for their child, but because the admin had completely consumed them. I watched fathers who hadn't slept properly in years because they were the only person who knew everything. I watched siblings who barely knew their parents because care coordination had taken over family life.
I made a decision. I wanted to make a difference. Not just at the bedside — but for the whole family, every single day.
MyCarePlan was built from hundreds of real conversations with real families. Every single feature exists because I watched a family struggle without it. This isn't a tech product with a care veneer — it is a care tool, built from the inside out.

The people who love someone most end up spending the least time loving them — because the system demands they become a case manager, a pharmacy technician, a medical secretary, and an NHS navigator all at once.

Every Tool. Every Feature. Explained.
Every feature was designed from real clinical experience — watching real families, understanding real pain points. Here is exactly what each one does and why it matters.
Track every medication, dose, time, stock level, and expiry date. Drug interaction checks and low-stock alerts mean nothing is ever missed.
Eliminates manual pill counting, written schedules, and missed doses
Every appointment — GP, specialist, therapy, hospital — in one place, with reminders, outcomes, and notes attached.
Ends the chaos of cross-referencing letters, diaries, and NHS texts
Log every piece of medical equipment — service dates, filter changes, supplier contacts, settings, and troubleshooting guides — all in one place.
No more missed service dates or frantic searches for supplier numbers at 2am
Purpose-built observation charts for CPAP, BiPAP, and ventilated patients — log SpO2, heart rate, pressure, VTE, leak, and more at each observation.
Clinical-grade monitoring at home with normal range alerts built in
Log glucose readings across reading types with doctor-set target ranges, trend charts, AI insights, and photo capture of monitor displays.
Replaces paper log books and provides consultant-ready trend data automatically
Track vital signs over time — blood pressure, temperature, pulse, weight, and more — with trend analysis and out-of-range alerts.
Catches deteriorations early before they become emergency admissions
Write and store structured clinical notes by category — respiratory, medication, behaviour, handover, concern — with flagging for urgent follow-up.
Creates a professional care record that can be shared with any healthcare professional
Store discharge summaries, letters, EHCPs, care plans, and any document that matters — categorised, searchable, and accessible from anywhere.
No more hunting through folders or missing documents at hospital appointments
Generate a secure, time-limited link to share selected data — medications, appointments, equipment — directly with any healthcare professional.
Eliminates hours spent re-explaining history at every new appointment
Invite family members, PAs, support workers, and healthcare professionals to share access — with granular control over what they can see and do.
Distributes the mental load so the primary carer is never the only one who knows everything
Create and store a comprehensive emergency plan — what to do, who to call, what the person needs — accessible instantly, even without signal.
Means anyone can step in confidently in a crisis, not just the primary carer
All emergency contacts — NHS 111, GP out-of-hours, specialist nurses, and personal contacts — one tap away, always.
Critical seconds saved when every second matters
Map out the full care day by time — personal care, meals, therapy, medication, rest — visible to every member of the care team.
Written once, shared with everyone — no more morning briefing calls
Artificial intelligence analyses health trends over time and surfaces patterns a busy carer might miss — presented in plain English.
Turns raw data into actionable insights, reducing the burden of health interpretation
Generate professional, consultant-ready reports at the click of a button — medications, vitals, glucose trends, and clinical notes all formatted.
Walk into any appointment fully prepared — saves hours of pre-appointment prep
Secure video appointments built into the app — connect with healthcare providers without the stress and exhaustion of travel.
Eliminates unnecessary hospital trips for routine check-ins
Log home physiotherapy exercises, track repetitions and progress, and share progress with the physio team between appointments.
Reduces the frequency of physio visits needed and keeps progress on track
Log SALT exercises, targets, and session outcomes — keeping progress visible and enabling more productive therapy appointments.
Nothing falls through the gaps between appointments
A built-in visual Makaton reference guide for carers, support workers, and school staff supporting non-verbal loved ones.
Reduces communication barriers and supports consistency across the whole team
Record and monitor dietary needs, restrictions, feeding plans, and nutritional targets — with the full picture in one place.
Keeps every carer, school, and provider on the same page about nutritional needs
Smart reminders for medications, equipment checks, and appointments — sent to whoever needs to know, not just the primary carer.
Nothing is forgotten. The mental load of remembering everything is lifted.
Critical care information — medications, emergency plans, contacts — available even without mobile signal or Wi-Fi.
Works on hospital wards, in rural areas, and during connectivity issues
Log observations and notes hands-free during care — because both hands are often already occupied.
Faster documentation with zero disruption to care delivery
A private, secure space to write journal entries, track mood, and process the emotional weight of caregiving — completely confidential.
Supports carer mental health and emotional wellbeing — often the most neglected part of the equation
Two-factor authentication, full audit logs, data access consent controls, and GDPR-compliant data handling throughout.
Your family's most sensitive data is protected to clinical-grade standards
Automated and manual data backups — your records are never lost, even if you change devices or something goes wrong.
Years of care records protected and portable
